host descriptionPatient with history of staphylococcal osteomyelitis and active substance use disorder presents with acute neck pain at his heroin injection site. Patient was observed to be bacteremic and treated medically with vancomycin plus cefazolin. Clinical course was complicated by extensive spinal osteomyelitis and paraspinal abscesses, multiple pulmonary opacities consistent with septic emboli and tricuspid valve endocarditis. Blood cultures consistently grew out two populations of Staphylococcus aureus, one methicillin-resistant and one methicillin-susceptible. Therapy was modified to daptomycin plus cefazolin resulting in pure methicillin-resistant cultures then clear blood cultures. The patient experienced relapse bacteremia two weeks later with daptomycin nonsusceptible methicillin-resistant Staphylococcus aureus and therapy was modified again to ceftaroline plus daptomycin. The following week the patient left against medical advice and was prescribed oral linezolid to complete therapy. Duration of initial bacteremia: 14 days.
